Responsibilities:
Responsible for managing and directing all resident and nursing care activities in the nursing center under the direction of the Director of Nursing
Provides for the delivery of professional nursing care in accordance with state regulations. Including: medication, administration, treatments and tube feedings.
Directs and supervises CNA’s and LPN’s regarding all areas of resident care including: nursing services, activities of daily living (ADL’s), activities and dining services.
Conducts nursing rounds in nursing center on assigned shift several times per day.
Monitors and maintains all resident records for completion, accuracy and regulatory compliance.
Monitors for, recognizes and documents changes in resident conditions and notifies physician and family and/or responsible parties as necessary.
Coordinates with the director of Nursing as to scheduling, orientation of assignments and duties for nursing staff.
Responsible for conducting and documenting resident assessments and re-assessments.
Develops implements and updates all resident care plants.
Responsible for overall regulatory compliance.
Develops and implements adequate staffing to insure proper resident care.
Communicates with physicians regarding orders and assures through follow up with physicians.
Liaison between family, significant other or responsible parties as it relates to resident care.
Implements established policies and procedures.
Maintains a safe work environment.
Participates as a member of the Infection Control Committee.
Skills Required:
Responsible for managing the day to day nursing care activities on the unit under the direction of the Director of Nursing. The Change Nurse is responsible for providing all nursing duties in his/her assigned shift including: administration of medications, treatments, assessments and documentation, supervising CNA’s, developing and updating care plans, communicating and following through with physician orders, being a liaison for family members and staff and assuring regulatory compliance.
